Self Determination Coaching

SDP Transitional Needs:

The facilitators provide individual program planning (IPP) support services that ensure the plan aligns with the individual's goals and needs. This includes coordination and review of the Initial Person-Centered Plan, as well as assistance during the IPP meeting to identify the potential SDP participant’s needs and goals, facilitating a smooth transition.

Ongoing Independent Facilitation

Ongoing facilitation services offer continuous support to help individuals navigate their SDP journey and achieve their goals. This includes managing their 12-month budget and spending plan, ensuring that clients receive the guidance they need to make the most of their resources.

Person Centered Plan

We specialize in person-centered planning services that are meticulously tailored to focus on the individual's strengths, capabilities, and preferences. This approach ensures that the client's unique needs and aspirations are at the forefront of the planning process, fostering a sense of ownership and empowerment.

Group Sessions

The facilitators conduct group support sessions for individuals with developmental disabilities, providing a safe space for participants to share experiences and learn from one another. These sessions foster community and connection, promoting personal growth and mutual support.

Family Consultation

Family consultation services are available to help families better understand and support their loved ones with developmental disabilities. These consultations are designed to empower families with the knowledge and tools necessary to foster a nurturing and supportive environment for their loved ones.
